Intensive In-Home Care

Intensive In-Home provides short-term, home-based services which offer emergency assistance 24 hours per day, seven days a week.  This service is designed to allow the family in crisis to remain safely together in their home, preventing out of home placement of the at-risk child/youth.  This service is also utilized to reintegrate a child back into the home and community after an out-of-home placement.

Mental Health Skill-Building

Mental Health Skill-building service is an intensive home-based program designed to help adults with mental illness or other dual diagnosis to live a successful and productive life in the most appropriate and least restrictive environment.  This service is focused on helping adults obtain the skills for living independently while managing mental health issues through goal directed training.

Theraputic Day Treatment

Therapeutic Day Treatment program is individualized for each child and is delivered in the child’s school-based setting.  This service delivers social and emotional learning strategies, psychotherapeutic interventions and opportunities to lean and use daily actions to enhance social, interpersonal skills, and increase instructional time in the classroom.  ICG has adopted a Truancy Protocol to help create consistency at school by supporting the student and family with strategies and services to foster a positive connection between the student and the school community.

Theraputic/Educational Monitoring

Therapeutic: This service is designed to provide targeted interventions through a detailed treatment plan where the child/youth depends dedicated one-on-one time with a caring mentoring to work on building a variety of life skills in the community.  Therapeutic mentoring is a time-limited and targeted service that can be a stand-alone service or as a grouping service to support other more intensive services to help ensure the child/you remains in the community.

Educational: This service is designed to improve the child/youth’s academic performance and school attendance through a detailed educational performance plan where the child/youth spends dedicated one-on-one time with a caring mentor to work on homework and building academic skills.

High Fidelity Wraparound Model Intensive Care Coordination (HFW-ICC):

ICC provides service coordination to support effective stabilization for children/youth at high risk for out-of-home placement and/or children/youth transitioning back into the community from an out-of-home placement.  HFW is the service delivery model for ICC.  HFW is a family-focused, strengths-based phasic approach to family stabilization.  The model provides structure for identifying family needs and creating a collaborative team approach among treating providers and stakeholders.  The model is designed to support families with identifying natural resources, which they can utilize to maintain stability upon discharge of services.  We strongly encourage this service to be paired with a Parent Support Partner to support the integrity and fidelity of the HFW Model.  Estimated length of this service should be between 9 and 15 months.

Other Services Available:

Life Skills Training (1:1): This service is designed to provide targeted skills detailed in a treatment plan that are based upon the completion of the Casey Life Skills Assessment.  This assessment assesses the behaviors and competencies youth need to achieve their long-term goals.  Identified life skills include, but are not limited to budgeting and paying bills, daily living activities, and using community resources.  It is recommended that clients receive a minimum of five hours per week.

Virtual Residential Services: Virtual Residential Services are designed to provide wrap around services (mixture of Intensive In-Home, Mentoring, Parent Support, and Crisis Intervention)  to the family that are strength-based, client-centered, and family oriented on an intensive short-term basis.  These services are offered by our highly trained counselors who will work collaboratively with the family in their natural environment which will allow them to maintain their child/youth in their home and community.
